Medical Insurance 1095-C forms COMING SOON!

EverTrue team members can expect to receive their 1095‑C tax form no later than March 2, 2026. This annual document outlines the health coverage offered to you and may be helpful when preparing your tax return. The 1095-C form is a way for employers to report each team member’s monthly medical benefits coverage eligibility, the cost of the lowest-premium plan offered at EverTrue, and confirms if you or your family members elected to carry coverage. Keep an eye on your mail and/or Workday, depending on how you have elected to receive this document.
