Wellness Screening Registration for 2025-26 Plan Year Opens Tuesday, July 1

Earn discounts on your 2026 medical premiums by registering for your annual wellness screenings – starting July 1.

To ensure you receive discounted medical premiums beginning January 1, 2026, all EverTrue team members and spouses enrolled in medical benefits as of July 1, 2025, or earlier, should register for their biometric health screening through Quest Diagnostics’ EverTrue wellness portal, My.QuestForHealth.com.

Be on the lookout for your Quest official welcome email to walk you through screening registration in the Quest portal. Please share with your spouse if they are enrolled in the medical plan.

 

Scheduling Your Appointment

First time participant

Follow the steps below to register with the Quest Portal:

  • Visit My.QuestForHealth.com

  • In the Create Account area (green box), enter your Registration Key: evertrue

  • Click the Register Now button

  • When prompted, enter your Unique ID: Employee ID (Spouses’ Unique ID will be the Employee ID with an “S” added to the end, e.g. 123456S)

  • Complete the screens that follow to finish the registration process

  • Once you’ve registered, you’ll arrive at the dashboard

  • Under Wellness Screening, select an option to schedule your screening

Returning participant

Metric

We are happy to offer the same five metrics to be measured this year, including: 

  • HDL Cholesterol

  • LDL Cholesterol

  • A1C hemoglobin (glucose)

  • Triglycerides 

  • Blood pressure

Earning the Wellness Credit

Participants only need to meet/pass three or more metrics to earn the wellness credit with no additional steps to take.

If a participant fails to meet at least three of the five metrics, they can still earn the wellness credit by taking a “next step,” such as having your healthcare provider complete the Physician Engagement Form. 

The Physician Engagement Form is currently the only way to satisfy the "next step" to earn the wellness credit if you fail to meet/pass 3 or more metrics. Quest discontinued their virtual care option in May. EverTrue will continue to search for a viable option as a replacement virtual care program. 

Screening Options

To receive your wellness credit, you must register through the Quest portal. Your three screening options include: 

  1. At a participating Quest lab (aka “patient service center”)

  2. At the employee’s own home using the “At-Home Screening Kit” 

  3. Submitting the required lab work provided by the employee’s own physician using the Physician Results form.

Fasting Required

Fasting for eight hours prior to your screening is required for all biometric screenings regardless of the method you choose.
